All Ceramic Fixed Partial Dentures

Porcelain, resins and porcelain fused to metal are the classical and most often used materials in fixed partial prosthodontics. The teeth and presently – dental implants, used as support gives us very adequate prosthodontic rehabilitation.

The patients’ need for esthetically sound dental treatment, along with the desire of dentists in search of better prosthodontic solutions, have created an industrial and technological revolution which has provided the dentist with better options. The developments have improved the biomechanical skills of these new dental materials. The search for better esthetic restorations has continued. New technologies are searched to produce better adaptations of dental crowns and fixed partial dentures to the abutments.

The search is towards obtaining a real esthetic improvement of fixed partial dentures, with metal free restorations. Structural modifications have been made in ceramic material to fulfiull the need considered to be part of an ideal restoration material from an esthetic and structural strength point of view.

The concept of CAD CAM milled crowns and bridges is based on doing the drawing and manufacture of densely sintered infrastructure aided by the computer to fabricate a total ceramic crowns. Instead of using the casting or the lost wax technique, CAD CAM system is based on the drawing of the die and production of the coping by the computer. CAD CAM system is used to fabricate a prosthodontic framework in alumina or zirconia, that will receive a compatible porcelain (coefficient of thermal expansion) to make a fixed partial denture.

Advantages of CAD CAM milled zirconia fixed partial dentures:
1. Excellent esthetics.
2. Metal free without losing strength.
3. Good shade stability.
4. Less laboratory time for the construction of zirconia framework.
5. Excellent adaptation to the prepared abutment.
6. Avoids the hand made aspects of convention FPDs.
7. All ceramics are essentially noncorrosive by nature.
8. All ceramic materials have low affinity to plaque.
9. Very low temperature conduction.

Disadvantages of CAD CAM milled zirconia fixed partial dentures:
1. Expensive equipment.
2. Special training for the technician.
3. Obtaining the Chameleon effect is difficult because of stark white color of zirconium dioxide ceramic framework.
